Medical Power of Attorney

Why it matters

When a sudden injury or illness leaves someone unable to speak for themselves, the next 48 hours move fast. Decisions about treatment, life support, and organ donation fall to whoever is legally authorized to make them. When a Medical Power of Attorney exists, that person already knows what their loved one would have wanted — because they had the conversation first.

The foundation exists, in part, because our family didn’t have that clarity in the hardest moment of our lives. We don’t want you to learn what we learned the way we did. We want you to have the tools, the conversation starters, and the forms — before you need them.

What is a Medical Power of Attorney?

A legal document that names one person you trust to make medical decisions on your behalf if you’re unable to. It only activates when you can’t speak for yourself — and it can be updated anytime.

What is an Advance Care Directive?

Written instructions about the care you would or wouldn’t want in specific medical situations — things like life support, resuscitation, and organ donation. It speaks for you when you can’t.
